What's the best place to get a run from to the Underworld?

I think you need to clarify what it is you're asking, but I'll take a stab at it. UW is accessed from Temple of the Ages, assuming you have favor. ToA is located a couple zones northwest of Bergen Hot Springs, which is northwest from Lion's Arch. Your character must be ascended to enter Underworld or Fissure of Woe, also.

What does "Ascended" mean?

Ascended or Ascension is the term for gaining the attention of the gods.

You need to complete the three desert missions Thirsty River,Elona Reach and Dune Of Despair then face your dopplegander.

Only once your ascended can yu gain entry into Fissure Of Woe and Underworld
